TERMS OF SERVICE


 

Phlebotomy Genie Mobile Service is an independent blood draw service that is offered as a convenience to the healthcare community. PGMS will come to your home, workplace, doctor’s office, company events, blood drive, senior and nursing facility to obtain your blood sample(s) by venipuncture or capillary and properly deliver or ship it to the laboratory of your provider’s choice. 

                               

PGMS is not in any way an acting diagnostic laboratory or physician’s office. Therefore we are not authorized to diagnose, treat or give medical advice to any person. 

 

PGMS is a private pay service. Blood draw services are provided and billed by PGMS. We do not bill your insurance for our service fee. Payments for our services are made directly to PGMS and must be made before services are rendered unless stated otherwise. Please be advised that all laboratory tests will be billed to your insurance company by the resulting agency. PGMS is not responsible for any outstanding balance or charges you may incur. For any billing questions or concerns contact your insurance company and/or resulting laboratory billing department listed on your statement. 

 

PGMS is not responsible for any ordering inaccuracies or laboratory testing errors. Please contact your physician’s office or laboratory for correction. In the event that a recollect is needed due to an error not made by PGMS there will be a recollect charge. Any recollection error caused by PGMS will be recollected at no charge to the patient.

PGMS is in full awareness of patient’s privacy, and we assure that all information will be kept confidential. Under no circumstances will your information be shared with any unauthorized personnel. PGMS does not receive nor have access to your laboratory results; you must contact the ordering physician or resulting laboratory.

PGMS will need the following before the blood collection process. complete lab order, insurance card, photo ID (will be checked in person) and service fee. Payments are invoiced and collected through Square Inc for Phlebotomy Genie Mobile Service. You will receive a link via email two hours before your appointment to process your payment. If you are self-paying the service fee and laboratory fee must be paid in full on or before services are rendered. To better prepare for your appointment all documents must be received in our office no later than 24 hrs prior to your scheduled appointment or your appointment will need to be rescheduled.  We do not accept verbal lab orders!!

PGMS is a convenient blood draw service provided to the healthcare community. Although each patient is very important to us, our scheduled appointments are a priority. We encourage you to take advantage of our online booking to guarantee your blood collection appointment. You will receive a courtesy call or text the day of your appointment when your technician is en route. For any reason you need to reschedule your appointment you may contact PGMS via email, text, or phone. Email: thephlebotomygenie@gmail.com Phone: (919) 438-1482.

Please be advised that a same day cancellation will result in a $15 charge. There is a $25 charge if our technician arrives at your destination and you are not there or you reschedule. Due to our technician's schedule they are not allowed to wait around.
